Base Salary: $103,713 to $110,813 (RN 3.1 to 3.3) or $74,007 to $96,681 (AHP 2.1 to 3.4)

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
ABOUT THE ROLE
The Aboriginal Health Practitioner (AHP)/ Registered Nurse (RN) provides high quality, comprehensive and culturally appropriate primary health care to Aboriginal and/or Torres Strait Islander people of the Greater Darwin Area. Supported by and supporting other health practitioners both within DDHS and the community. This role is crucial to maintaining cultural integrity and advocates strongly for our clients.
WHAT YOU WILL DO
- Be a first point of contact for clients attending the DDHS Clinics, including completing an initial diagnosis, treatment, and referrals as necessary (SODAF) with guidance from a senior clinician, work as per CARPA guidelines:
- Screen patients and treat according to standards and patient's journey.
- Provide advice about healthy lifestyles, e.g., nutrition and physical activity, alcohol consumption and quit smoking programs through brief interventions.
- Identify 'at risk' clients, e.g., older people, youth, and children.
- Monitor/ maintain client group recalls.
- Perform child and/or adult health checks
- Refer to complimentary DDHS programs, link closely with local communities and support existing primary care services.
- Provide early intervention, coordinated care and information and referral services.
- Support General Practitioners and other medical staff including visiting health professionals and specialists.
- Ensure all information given is provided in a way that the patient, carer, and community understand.
- Obtain feedback from the patient, carer, and community to check that the information and advice was correctly understood.
- Increase knowledge and use of chronic disease self-management options including medication compliance.
- Improve follow up care for patients with a chronic disease and the identification of support needs of patients.
- Provide day to day health care service activities within the context of Primary Health Care in a professional, confidential, and culturally safe manner.
- To provide cultural brokerage between patients and non-indigenous staff to provide culturally safe and appropriate advice and support to contribute to better health outcomes for Aboriginal and Torres Strait Islander people.
- Contribute to the effective management of patient care plan systems, including participating in-patient care conferences, managing patient follow-up procedure.
SELECTION CRITERIA
Essential
- Bachelor of Nursing and/or Certificate IV in Aboriginal & Torres Strait Islander Primary Health Care (Practice).
- Current registration as an AHP/ Nurse with AHPRA.
- Demonstrated understanding and/or experience in providing Primary Health Care, utilising standard treatment manuals.
- Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions and relevant management strategies.
- Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and well-being of Aboriginal and Torres Strait Islander people.
Desirable
- Remote health experience and/or emergency care experience.
- Experience working in an Aboriginal Community Controlled Health Service or similar environment.

EMPLOYMENT IS SUBJECT TO
- Willing to undergo a Police Check.
- Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
- Current driver's licence.
- Current CPR certificate or the preparedness to gain one within 6 months of commencement.
- Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR) be submitted.
