About the Role

Isaac Navicare is seeking an experienced and compassionate Mental Health Care Navigator to connect individuals in Clermont and the Bowen Basin with the right mental health and support services, whether in-person, virtual, telehealth, or community networks.

This is a 12-month fixed term based at our Clermont hub.

What You will Do

You will be the first point of contact for people seeking help with mental health. Your role is to guide them to the right support, whether that’s local services, virtual mental health providers, or community networks. You’ll build connections, coordinate access, and ensure each person feels heard, supported, and safely guided to the care they need.

You will also play a vital role in assessing when a person may be at higher risk and acting quickly to connect them with emergency services, crisis support or other support, if needed.

Patient advocacy & coordination – guide individuals and families to accessible, timely mental health support and care.

Support - provide necessary information to individuals about the services available to support their individual needs and advise of possible wait times for services and support programs.

Service liaison – collaborate with local providers, crisis teams, telehealth services, GPs, and community groups.

Triage support – assess for urgent risk and coordinate crisis or emergency services when needed.

Case management – maintain clear documentation and follow-up to ensure seamless care transitions.

What You Bring

Essential

Certificate, Diploma or tertiary qualification – nursing, social work, psychology, behavioural science, human services or equivalent experience in mental health.

Proven experience in coordinating mental health services, especially in rural/remote settings.

Demonstrated skills and ability to coordinate access to services and support for help-seekers and their family.

Strong stakeholder relationship-management skills, including with the community, organisations, general practice, help-seekers and other stakeholders.

Excellent interpersonal and communication skills, including the ability to interact effectively with multi-disciplinary medical, nursing and allied health staff, and community groups.

Excellent autonomy, judgement, time management, and communication abilities.

Proficiency in MS Office and client database systems.

Working with Children Blue Card, or eligibility for a Blue Card

Mental Health First Aid certification, or willingness to obtain and maintain this.

Current First Aid certification including CPR, or willingness to obtain and maintain this.

Current Driver’s License.

Desirable

5+ years’ relevant experience.

Higher degree in relevant discipline.
