About the Role
The Team Leader Service Response oversees the team receiving day-to-day incoming enquires relating to Council aged care services delivered under the Commonwealth Home Support Program and Home and Community Care Program for Younger People. The Team Leader will be required to:
• Lead, support and develop the Service Response Team to deliver optimal client outcomes and excellent customer
• Maintain required service levels and coverage across a team to ensure smooth day-to-day operations
• Oversee incoming referrals via the My Aged Care (MAC) portal
• Respond to and monitor client enquiries, call backs and client related concerns or issues
• Work collaboratively to foster a quality approach and practice to achieve improved client outcomes
• Operate software systems to compile and analyse service-related data for outcome measures and reporting
• Maintain sound knowledge of the needs of older adults and relevant implications for the provision of services and supports to diverse communities
Skills & Experience
• Tertiary qualification in human services (or related discipline)
• Experience in leading a team to deliver community-based services to clients
• Excellent communication, interpersonal and problem-solving skills
• Experience with client intake, assessment or care planning
• Knowledge of relevant legislative requirements pertaining to Aged Care and the Aged Care Act
• Current Working with Children Check card
• Proof of Covid vaccinations
