About the Role
We are seeking a highly skilled and values-driven Clinical Care Coordinator to join CaSPA Care on a full-time basis. Based primarily at Emerald Hill Residence, the role also provides support to residents and staff at our South Port Residence.
As a senior member of the clinical team, you will play a vital leadership role—working alongside the Director of Nursing to deliver safe, effective and person-centred care in line with the Aged Care Quality Standards.
This role is ideal for a Registered Nurse with strong clinical expertise, a commitment to continuous improvement, and the ability to coach and mentor others while contributing to quality and governance activities.
Key Responsibilities
Lead and coordinate clinical care delivery in line with individualised care plans
Undertake specialised assessments and oversee complex care including diabetes, wounds, cannulas, and PEGs
work with the team to review clinical audits, incidents and care planning processes to support quality improvement
Provide education, coaching and support to nursing staff and homemakers
Liaise with residents, families, GPs, and allied health professionals to coordinate care
Contribute to preparation and completion of AN-ACC assessments and reporting
Work alongside the IPC Lead to implement infection prevention strategies and respond to outbreaks
Collaborate with the Director of Nursing to achieve quality care outcomes
What You’ll Bring
Current AHPRA registration as a Registered Nurse
3–5 years’ experience as a Registered Nurse in Australia (aged care experience highly regarded)
Proven clinical leadership capability
Excellent communication, documentation and organisational skills
Ability to lead and support projects related to clinical care
A commitment to delivering person-centred, inclusive care
