About the Opportunity

We are seeking an experienced, forward-thinking and compassionate leader to join our Community & Integrated Care Division as the Manager - Ambulatory Aged Care Programs. In this strategic and operational leadership role, you will be responsible for the coordination and management of a suite of programs that support older adults to remain in or return to the community safely and independently. These programs are critical in supporting transitional care, reducing avoidable hospital admissions, and promoting functional recovery.

Programs under your leadership will include:

  • Transition Care Program (TCP) - both residential and home-based models
  • Geriatric Evaluation and Management in the Home (GEM@Home)
  • Restorative Care Services
  • Residential In-Reach Services (RIR)
  • NDIS and Statutory Residential Services for Older Adults

Key Responsibilities

  • Provide day-to-day operational management and strategic leadership across multiple programs and sites
  • Drive program development, growth, and continuous quality improvement initiatives
  • Lead and support multidisciplinary teams, fostering a culture of collaboration and excellence
  • Oversee program budgets, workforce planning, data collection and reporting
  • Ensure compliance with all relevant clinical standards, aged care legislation, and funding requirements
  • Develop and maintain strong partnerships with internal and external stakeholders, including My Aged Care, residential aged care services, community providers, and the Department of Health
  • Act as a key advisor to leadership on aged care and community program delivery and integration
  • Champion a person-centred approach to care that supports older adults to achieve their goals and maintain their independence

Skills & Experience

  • Tertiary qualifications in Nursing, Allied Health, Health Administration, or a related discipline
  • Current AHPRA registration
  • Extensive experience managing aged care or community-based health programs
  • Demonstrated experience in leading and supporting multidisciplinary teams
  • In-depth knowledge of aged care reforms, My Aged Care, and Commonwealth Home Support Program (CHSP), NDIS and/or TCP funding guidelines
  • Strong financial and operational management skills, including budgeting and workforce planning
  • Exceptional interpersonal, negotiation, and stakeholder engagement skills
  • A strategic mindset with the ability to lead change and influence service improvement across complex systems
