About the Role
We're growing and looking for a passionate, organised, and skilled Support Coordinator to join our team! You'll work alongside participants, families, and service providers to bring NDIS plans to life. Your focus will be on understanding each client’s goals and ensuring the right supports are in place to help them thrive.

Key Responsibilities
Assist participants to understand and implement their NDIS plans
Connect clients with appropriate services, supports, and community options
Coordinate a range of funded and mainstream supports
Monitor progress and adjust supports in response to changing needs
Build positive, respectful relationships with participants and stakeholders
Maintain accurate, timely notes and documentation

About You
You are person-centred, proactive, and highly organised
You have experience with the NDIS or disability/community sector
You understand Support Coordination, or you're eager to learn
You thrive in a flexible, collaborative team environment
You are tech-savvy and comfortable working remotely or on the road
You hold (or are willing to obtain):

Current NDIS Worker Screening Check

Police Check

First Aid Certificate

Driver’s Licence and reliable vehicle
