Youth Homelessness is often hidden, with young people couch surfing between extended family and friends due to relationship breakdown in the family home, overcrowding, domestic and family violence, neglect, unsupported mental health, financial issues, teen pregnancies, young people identifying as LGBTIQA+, cultural or religious differences, AOD use, leaving OOHC and Juvenile Justice. Our young people are deserving of a safe, supported and encouraging environment to develop independence and manage their first tenancy. Role:

The Early Intervention and Prevention Program (EIP) is a Specialist Homelessness Service (SHS) that assists young people and young families aged 12-25 who are homeless or at risk of homelessness across the South West Sydney Area District. We support young people and young families to resolve their homelessness through internal and/or external accommodation options. We assist young people through case management to pursue progress in areas such as education, training and employment; financial management, living skills, parenting education, cognitive functioning/disability, health/mental health and wellbeing; providing brokerage, AOD support, advocacy and accessing other community and specialised support services. The program aims to reduce the likelihood of young people entering or remaining in the child protection and out-of-home care systems.

The Early Intervention and Prevention Team leader will lead the team of case workers to assist young people in improving their quality of life, safety and wellbeing; and to achieve access to stable, longer term housing options at the conclusion of their time in the program. The EIP Team Leader will provide exemplary leadership, supervision, support and mentoring to the team enabling the delivery of consumer – driven service excellent in accordance with organisational strategy, values and service principles whilst meeting our legislative obligations and funding requirements.

The EIP Team Leader will assist the team in linking young people with accessing support, maintain contacts with first to know agencies such as schools and health services as well as promoting awareness of youth homelessness. Provide coordination of supports for young people with multiple or complex needs to deliver integrated care plans and provide interventions to support young people to remain in safe, stable and sustainable accommodation.

Responsibilities Include:

  • Direct, coordinate and supervise the Early Intervention and Prevention Program including all referrals, assessments, care planning and reviews
  • Provide leadership supervision and support to create and maintain a high-performance culture.
  • Share expert, quality and responsive case management advice, guidance and develop skills and knowledge and builds capacity within the Early Intervention and Prevention Program
  • Monitor and evaluate service delivery, including but not limited to regular internal audits of client files and services.
  • Manage key resources including staffing and clients to ensure optimal service delivery.
  • Responsible for the performance of the Early Intervention and Prevention Program against key performance indicators, ensuring that all team members comply with documentation and statistical data reporting requirements
  • Contribute to the development, monitoring and review of policies and procedures pertaining to consumer safety and quality services.
  • Monitor program budgets
  • Provide Case Workers with timely advice, support and information based on best practice, risk management, Practice Guidelines and CORE Policy and Procedures
  • Participate in accreditation standards and practices activities in conjunction with the Program Support Manager.
  • Act as an advocate for young people ensuring that their needs are accurately understood and met and that young people are involved in any decision-making processes which affect their lives.
  • Provide direct financial assistance via brokerage to young people who are homeless or at risk of homelessness within a case management plan, ensuring it is delivered in line with funding agreements and program guidelines.
  • Develop and maintain positive and effective professional working relationships with key partners, internal and external stakeholders, other SHS programs, local community services, ‘first to know’ services and mainstream services who are key to providing support to young people.
  • Maintain privacy and confidentiality of information pertaining to clients and staff.
  • Ensure the accurate recording of all statutory and program related information requirements for client records and case planning, including CIMs reporting, case note sharing, records management, statistical and other data collection.
  • Be involved in action which positively influences the policies and social factors that affect at risk young people.
  • Review and maintain all Service Agreements and ensure that Service Agreements dates and milestones are achieved, in line with Agreement for Funding of Services.
    Actively marketing and promoting CORE and the Early Intervention and Prevention programs to new prospective Participants and service providers.

Selection Criteria

  • Tertiary qualifications or higher in Social Work, Social Science, Community Welfare or community sector or equivalent and 2 years’ minimum experience.
  • Demonstrated experience in leading a team and service/ project co-ordination in a community setting including: Priority setting, problem solving skills, skills in implementing agreed plans and strategies, monitoring outcomes, supervising staff and team building.
  • Demonstrated awareness of child protection issues, with the capacity to identify levels of risk requiring statutory child protection intervention.
  • Superior case management skills and previous experience in a similar role, ability to manage case load.
  • Proven knowledge of the Early Intervention and Prevention framework and other relevant legislation.
  • Highly skilled in communication, mediation and conflict resolution skills.
  • Demonstrated competence in networking, community liaison and community capacity building; working with a range of appropriate professionals in youth related services.
  • High level of written and computer literacy, demonstrated knowledge of and experience using data collection systems, tools and data evaluation processes, including the ability to complete comprehensive reports.
  • Capacity to work with minimal supervision with a high ability to meet deadlines, manage competing priorities and tasks.
  • Demonstrated ability to work within professional boundaries, integrity and ethical standards.
  • Possess a valid NSW Working with Children Check and Australian Federal Police Check.
  • Possess a current NSW driver license.
