About the role
This is a full-time opportunity to join the Groote Archipelago Regional Council as an Aged Care and Disability Services Coordinator based in Umbakumba, NT. In this pivotal role, you will be responsible for coordinating and delivering essential aged care and disability support services to the local community, playing a key part in enhancing quality of life for some of the region's most vulnerable residents.
What you'll be doing
Manage the delivery of high-quality aged care and disability services, including providing case management and client support as well as the development, review, and monitoring of individualised care plans.
Coordinate the rostering and supervision of Aged Care and Disability Support workers to ensure efficient and effective service delivery.
Liaise with clients, their families and other stakeholders to understand and meet their unique needs.
Maintain detailed client records and ensure compliance with relevant legislation and funding guidelines.
Contribute to the ongoing development and improvement of service offerings.
Represent the Council at community events and engage with local organisations.
What we're looking for
Relevant tertiary qualifications in aged care, disability services or a related field.
Minimum 3 years' experience in a similar coordination or management role within the community services sector.
Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers and stakeholders.
Demonstrated problem-solving skills and a commitment to delivering high-quality, person-centred care.
Sound understanding of the aged care and disability support frameworks, including relevant legislation and funding requirements.
Proficiency in using client management systems and MS Office applications.
A current driver's licence and willingness to travel within the local region.
