Job Description

The Case Manager reports to the Manager of Aged & Disability Services and is responsible for assisting clients to live and participate in their community while maintaining a quality of life that maximises their potential through increased choice and decision making about their care. The position liaises closely with the home care team, including other Case Managers, Registered Nurses, Team Leader, Care Workers and the Manager of Aged & Disability Services.

Key Selection Criteria:

- Relevant qualifications and/or substantial experience in community aged care, particularly within a consumer-directed environment, encompassing both Home Care Packages (HCP) and the Commonwealth Home Support Program(CHSP).
- Proven track record of conducting client assessments and crafting tailored care plans that meet individual needs.
- Experience in managing dynamic scheduling in a fast-paced work environment.
- Proficiency in recording and maintaining accurate electronic records.
- Strong organizational skills to prioritize tasks and meet competing deadlines efficiently.
- Cultural competence and sensitivity to address the diverse needs and backgrounds of consumers.
- Initiative, discretion, and sound judgment in coordinating client care and services.
- A high level of information technology literacy, including proficiency in Microsoft Office Suite (Outlook and Excel) and client management systems.
