Clinical Care Partner – Support at Home

We are looking for a compassionate and experienced Clinical Care Partner to join our growing team. You’ll play a vital role in supporting older people to live safely and independently at home through high-quality clinical care, care planning, and collaborative service delivery.

What you'll do:

Conduct comprehensive clinical assessments of participants

Deliver direct clinical care (e.g., medication administration, wound care, continence and skin integrity support)

Develop, review, and evaluate care plans and budgets within required timeframes

Monitor changes in participant health and adjust care plans accordingly

Educate participants, families, and support staff on health and care strategies

Work closely with Care Partners and other professionals to coordinate care

Maintain accurate clinical documentation and risk assessments

Lead and mentor care staff to ensure safe, effective service delivery

Support referrals to external health and specialist services

Participate in an after-hours on-call roster (2–3 times/year with allowance)

Assist with care worker competency assessments and skills checks

About you:

Must have a Bachelor Degree in related field.

AHPRA registration as a Registered Nurse, or other qualified health professional

Clinical experience in aged care, home care, or community health

Strong understanding of person-centered, wellness and rabblement approaches

Excellent communication, documentation, and time management skills

Ability to work collaboratively and independently within scope of practice

Passion for improving the quality of life for older Australians

Must have a Driver license, NDIS Check and WWCC
