We are a well-established General Practice located in Kangaroo Point seeking an NDIS Registered Nurse to lead a new division of our business.
We provide a respectful, positive and friendly work environment. The suitable candidate will need to have excellent communication and administrative skills. Your primary responsibility will be to provide support to patients and doctors.
The successful candidate will be eligible for a bonus structure to reward your hard work.
Availability:
Monday to Friday, flexible hours during 8 - 5pm, 38 hours per week.

Key Responsibilities
Develop, modify, and evaluate care plans aligned with clients’ health needs and personal goals.
Support participants in understanding the basics of their NDIS plans and available supports.
Assist in linking participants with appropriate service providers, community resources, and culturally safe supports.
Learn to develop simple action plans in collaboration with Support Coordinator, focusing on short-term goals.
Provide administrative support, including maintaining case notes and ensuring compliances with policies.
Observe and participate in meetings with participants, families, and stakeholders to gain hands-on experience.
Provide clinical leadership, guidance, and supervision to the care team.
Monitor and evaluate the effectiveness of nursing care plan implementation.
Deliver training, supervision, and support to ensure care is delivered within scope.
Conduct home visits to review care plan effectiveness, support the team, and strengthen client relationships.
Perform clinical and risk assessments based on clients’ health needs and goals.
Oversee medication management.
Collaborate with GPs, care coordinators, and allied health professionals to ensure seamless care delivery.
Ensure compliance with accreditation and clinical standards.
About You
Bachelor of Nursing with AHPRA registration (no restrictions) or similar qualifications
Previous NDIS / case management experience is essential
First Aid/CPR Certificate
Strong administration, communication, and time management skills.
Driver’s license and own vehicle.
Current National Police Clearance or ability to obtain.
Working with Children's Card (Blue Card) or be eligible to apply.
Disability Worker Screening (Yellow Card) or be eligible to apply.
NDIS Worker Screening (or willingness to obtain).
