Manager - Hospital Without Walls

Grampians Health is proud to launch its innovative Hospital Without Walls service—a transformative model of care designed to deliver high-quality, multidisciplinary healthcare to patients in the comfort of their own homes and communities.

We are now seeking an experienced and visionary leader to take on the role of Manager - Hospital Without Walls. This is a unique opportunity to be at the forefront of healthcare innovation, overseeing a service that integrates Grampians at Home, Grampians Watch, and Post Acute Care.

About the Role

The Manager will play a pivotal role in the day-to-day operational leadership and strategic direction of the Hospital Without Walls program, working closely with Clinical Directors and other senior leaders to drive service excellence.

You will be responsible for ensuring high-quality, safe, person-centred care across the service, supporting a positive workplace culture, and embracing innovation to enhance community-based healthcare delivery across the Grampians region. This role spans care for patients of all ages and diagnoses, with a strong focus on collaboration, adaptability, and continuous improvement.

Key Responsibilities

• Lead and manage daily operations of the Hospital Without Walls service in alignment with NSQHS standards and relevant healthcare frameworks.
• Provide strategic and operational leadership across integrated programs: Post Acute Care, Grampians Watch, and Virtual Care.
• Contribute actively to the extended leadership team within Medical and Clinical Programs.
• Promote and foster a culture of innovation, collaboration, and clinical excellence.
• Oversee workforce planning, service delivery, and clinical governance to ensure safe, high-quality outcomes.

• Extensive experience and advanced clinical knowledge in community nursing, home-based care, and managing complex health needs.
• Demonstrated leadership and supervisory experience within a multidisciplinary team environment.
• A strong commitment to person-centred, high-quality, and safe care.
• Proven skills in financial management, data analysis, and the ability to generate clear and insightful reporting.
• A forward-thinking approach and a passion for shaping the future of healthcare delivery.

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant (ZA7-ZA8) which ranges from $77.75 per hour.
