We are seeking an experienced health professional, to join our fabulous team of 25 MND Advisor and Support Coordinators who support people diagnosed with MND across Victoria and Tasmania. This role will primarily support clients within the Barwon Region and will have the flexibility to work from home.

In this role you work with those impacted by Motor Neurone Disease to:

provide advice and support to assist people impacted by MND to establish an understanding of the disease and its impact

undertake holistic and timely assessment

identify, refer and negotiate services to address current and future needs

provide Support Coordination for NDIS participants and help people navigate aged care and palliative care services.

The successful applicant will have:

experience in working with people living with a progressive degenerative neurological disease

excellent knowledge of support and care planning, assessment and referral in the disability and/or aged care sectors

a “can do approach” with a demonstrated client focused approach to all work activities

A relevant tertiary qualification in a health/welfare related discipline

This role offers diversity and variety working across NDIS for those clients under 65, and My Aged Care for those over 65.

We require the successful applicant to have experience working with people living with a progressive degenerative neurological disease, however, are able to offer extensive training about Motor Neurone Disease itself.
