We are looking for a caring, empathetic but experienced staff member, who can work autonomously, safely and confidently with Aged Care clients in their own homes. What You'll Do:

In this role, you will work closely with the Team Leader, OTs and Allied Health Assistants to deliver person-centred, goal-directed care plans for clients referred through the My Aged Care program. Your responsibilities will include:

• Conducting home-based clinical assessments to evaluate clients' needs and functional abilities.
• Have recent experience in HCP, CHSP and STRC programs
• Attend a community centre where you will work with/supervise allied health staff.
• Developing and reviewing care plans that focus on reablement and enhancing clients' independence.
• Collaborating with clients and their families to set achievable goals and ensure their needs are met.
• Providing assessments for home modifications and assistive equipment to improve clients' quality of life.
• Maintaining accurate clinical documentation in line with organisational standards.

What You'll Bring:

To thrive in this role, you should have:

• A Bachelor's degree in Physiotherapy or an equivalent qualification.
• Registration with AHPRA
• Experience working with older adults, including those from diverse backgrounds and those living with dementia.
• Strong communication skills and the ability to engage clients holistically.
• Excellent organisational skills to manage your own caseload effectively.
• Be available for full days of work (Mon-Friday)
