What you'll be doing
Intouch is a community-based service within a multidisciplinary team that provides care as a telehealth service. The purpose of the InTouch Registered Nurse, urgent care for older people in Residential Aged Care Facilities (RACF), it to provide holistic nursing care to older patients residing in a RACF, who are at risk of hospitalisation due to complex care needs.
The objective of the position is to provide a single point of contact for RACF and NSW Ambulance staff to maximise the health outcomes for patients through conducting health assessments, providing care coordination between health care providers, assisting patients in accessing services and communicating health information.

Essential Criteria:
- Registration: Current Registered Nurse Division 1 National Registration APRHA
Selection Criteria:
- Current registration as a Registered Nurse with the Nurses and Midwives Board of Australia.
- Previous relevant clinical experience in providing complex care as part of a multidisciplinary team.
- Demonstrated effective interpersonal skills and ability to work effectively with a range of health professionals/service providers.
- Demonstrated experience in care coordination and care navigation, including effective development of care plans and identifying appropriate referral pathways.
- Demonstrated experience facilitating education and self-management support for clients with complex care needs.
- Experience and working knowledge of Information Technology systems currently used in providing patient care in WSLHD, such as CERNER, iPM and CHIME. Experience and knowledge working with Microsoft applications such as Word, Excel and Outlook.
