About the Role

As a Support Worker with Five Rivers Care, you'll provide essential personal care and daily living assistance to their valued participants in their homes and within the Mandurah community. Your compassionate support will enable individuals to maintain their independence, enhance their well-being, and fully participate in life.

Key Responsibilities (may include, but are not limited to):

Assisting with personal hygiene routines (showering, bathing, grooming) with respect and sensitivity.

Supporting individuals with dressing and undressing.

Assisting with mobility and transfers (e.g., in and out of bed, using mobility aids).

Supporting with continence management.

Administering medication as per care plans and training.

Assisting with meal preparation and eating.

Light domestic assistance to maintain a clean and safe living environment.

Providing emotional support and companionship.

Supporting community access and social participation.

Accurate and timely documentation of support provided.

What We're Looking For

Ability to maintain professional boundaries and respect privacy and dignity.

Reliability, punctuality, and a strong work ethic.

Ability to work independently and as part of a supportive team.

Current First Aid and CPR certificates (or willingness to obtain).

Valid Australian Driver's Licence and reliable, insured vehicle.

Current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).

Relevant qualifications (e.g., Certificate III in Individual Support, Aged Care, or Disability) highly regarded, but not essential for compassionate and eager learners.
