Advance Diversity Services (ADS) is a leading non-profit organisation that empowers individuals, families and communities from diverse backgrounds to fulfil their potential. We provide quality community services, including in-home aged care, social groups for older people, disability services, settlement support for migrants and refugees, emergency relief, and community development initiatives.
Our service area is South Eastern Sydney, including Bayside, Georges River and Sutherland Shire local government areas. ADS offices are located in Hurstville, Rockdale, Bexley and Sutherland. Initially established in 1981, ADS has strong connections and experience in working with local communities, organisations and services, different levels of government, businesses and stakeholders.
Our role as a service provider is to ensure our consumers are treated with dignity and respect, can maintain their identity and culture, and can make informed choices about their care and services, to live the life they choose.
We are looking for an experienced Team Leader to join our Care Finder program, which aims to provide specialist and intensive support to the Care Finder target population, to understand and access aged care services and other relevant supports in the community.
Skills, Experience and Knowledge and Key Attributes (Selection Criteria):
Relevant tertiary qualifications in social work, human services, aged care, community services or health.
Demonstrated experience in leading and managing a team.
Demonstrated experience in managing programs and funding contracts.
Demonstrated Understanding and Commitment to a person-centred approach, inclusive practice and strength-based approach.
Sound knowledge of My Aged Care systems and support services that are available in the local community.
Experience in working with CALD communities.
Ability to ability to build rapport and trust with clients and stakeholders.
Excellent communication, negotiation, problem-solving and casework skills.
Sound computer literacy, including collecting and reporting data.
Full driver’s license and ability to use your own insured vehicle.
Current Police Check.

Conditions:
Social, Community, Home Care and Disability Services Industry Award 2010 (MA000100) NSW; Base hourly rate from $53.50/hour, plus 11.5% superannuation and salary packaging.

Employment is subject to a satisfactory Police Check.
