Our office is in Heidelberg. The role will be a combination of being based at office and out in the community.

Key Roles & Responsibilities
Intake and onboarding of clients.
Conduct comprehensive assessments of clients’ health, social, and functional needs.
Develop individualised care plans based on assessment outcomes, considering clients’ preferences and goals, review accordingly.
Strong relationship management skills with external stakeholders.
Assist clients in making informed decisions about their care options.
Manage clients' Home Care Package funds, ensuring services are delivered within budget.
Assist clients in understanding their financial entitlements and responsibilities.
Ensure documentation complies with regulatory and organisational standards.
Stay informed about best practices in home care management and relevant regulatory changes.
Required Skills, Knowledge & Experience
Essential
Degree in nursing or a related field and at least 4 years of experience in a similar role in a home/community aged care setting.
Current AHPRA Registration.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work collaboratively with multidisciplinary teams.
Knowledge of healthcare systems, policies, and regulations.
Proficiency in electronic health records (EHR) and care management software.
Strong time management skills.
Current Driver's License.
Desirable
Strong understanding of self-management strategies within home care.
Basic knowledge of upcoming home care changes.
