We are currently seeking a passionate and experienced professional to join our dynamic team in a dual role as both NDIS Support Coordinator and Community Access Team Leader.
The Role:
As an NDIS Support Coordinator & Community Access Team Leader, you will play a pivotal role in supporting clients with their NDIS journey, ensuring they access the services, supports, and resources needed to lead a fulfilling and independent life. Additionally, you will lead a dedicated team focused on community access, helping individuals engage in meaningful activities and integrate into their local communities.

Key Responsibilities:
NDIS Support Coordinator
• Work directly with NDIS participants to develop, implement, and monitor their NDIS plans.
• Provide guidance on accessing appropriate services and supports that align with participants' goals.
• Assist participants in navigating the NDIS system and understanding their rights and responsibilities.
• Build and maintain strong relationships with service providers, families, and stakeholders.
• Coordinate and monitor service delivery to ensure participants receive the highest level of care and support.
Community Access Team Leader
• Lead and manage a team of community access workers, ensuring the delivery of high-quality services to participants.
• Develop and implement programs and initiatives that support individuals in accessing community activities and opportunities.
• Foster a person-centred approach within the team, encouraging active participation and inclusion in local events and services.
• Provide ongoing supervision, training, and support to team members, promoting professional development.
• Monitor and assess the effectiveness of community access programs and make improvements where necessary.
Skills and Experience:
• Previous experience working as an NDIS Support Coordinator or in a similar role within the disability sector.
• Proven leadership experience, with the ability to motivate and manage a team.
• Strong understanding of the NDIS framework, policies, and processes.
• Excellent communication skills, with the ability to build rapport with clients, families, and external stakeholders.
• Demonstrated ability to work independently and manage a diverse workload.
• Passion for empowering individuals with disabilities and helping them achieve their goals.
• Relevant qualifications Cert IV in Disability, Social Work, or similar preferred
