Management and Accountability
Direct accountability is to the Home Support Assessor Lead

Key Responsibilities
Assessment Coordination:
Triage, coordinate, and prioritise home support assessments based on client needs and urgency.
Monitor assessment timelines to ensure they align with organisational standards.
Complete assessments and related documentation accurately and promptly using the My Aged Care online portal.
Assessment Quality Assurance:
Conduct assessments using standardised tools and procedures to ensure consistency and quality.
Review assessment reports for accuracy, completeness, and adherence to regulatory requirements.
Provide feedback to assessors and facilitate continuous improvement in assessment practices.
Client Engagement and Support:
Act as a point of contact for complex or escalated client cases.
Collaborate with clients, their families, and caregivers to understand support needs and preferences.
Advocate for clients to ensure they receive appropriate and timely home support services.
Documentation and Reporting:
Maintain comprehensive and up-to-date records of assessments, including client information and service plans.
Provide a monthly report to the Accounts department of all assessments undertaken during the reporting period
Prepare regular reports on assessment outcomes, trends, and service delivery metrics.
Ensure confidentiality and security of client information in compliance with privacy regulations.
Requirements:
Qualifications in a Healthcare background ie. Registered Nurse, Social Worker, Physiotherapist & registered with AHPRA or own relevant professional body where relevant.
Current Australian driver’s license with own reliable, fully insured vehicle
Demonstrated expertise in using the Integrated Assessment tool to assess home support services.
In-depth knowledge of aged care assessment frameworks and regulatory requirements.
Proven ability to work independently and take initiative, with minimal supervision, while still collaborating effectively with team members when needed.
Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse stakeholders.
Strong time management skills, with the ability to prioritise tasks and manage multiple responsibilities simultaneously.
Strong analytical and problem-solving abilities, with a client-centered approach to service delivery.
Proficiency in using assessment tools and computer applications for documentation and reporting.
Valid National Criminal History Check and Working with Children Check or willingness to obtain one.
Proof of annual Flu vaccination
Work Environment: This position is based in Seddon with travel required for client visits or team meetings. The role may involve flexible working hours to accommodate client needs and team schedules.
