About the Role
As a Support Coordinator, you will assist NDIS participants in understanding and implementing their plans, connecting with services, and building their capacity to manage supports independently. This role starts at 8 hours per week, with the opportunity to grow as our client base expands.

Key Responsibilities

· Coordinate and implement NDIS plans in collaboration with participants

· Connect clients with appropriate services and supports

· Monitor progress and adjust supports as needed

· Maintain accurate documentation and reporting

· Advocate for participant needs and goals
