Community Services Case Officer

Evolving Care is seeking a dedicated and experienced Community Services Case Officer to join our growing team. In this vital role, you’ll play a key part in coordinating high-quality, person-centred support for NDIS participants throughout South East Queensland.

About the Role:

As our Community Services Case Officer, you will:

Coordinate and oversee participant onboarding, assessments, and care plans

Provide hands-on practical and emotional support to individuals and families

Build strong relationships with key stakeholders, including Support Coordinators and NDIS Planners

Deliver timely, effective services that evolve with participant needs

Promote community connection through outreach programs, support initiatives, and local engagement

What We’re Looking For:

Minimum 2 years’ experience in a care, community services, or participant support role

Certificate IV in Disability, Community Services or similar (highly regarded)

A current driver’s licence and reliable vehicle

NDIS Worker Screening Check (or willingness to obtain)

Strong communication, organisational, and problem-solving skills

A passion for improving lives through inclusive, respectful, and empowering service delivery
