The Role
We’re seeking experienced and passionate Disability Support Workers to provide in-home and community-based care for children and adults with varying needs. While we’re not exclusively recruiting for high-intensity roles, we strongly welcome applicants with experience in complex care, including PEG feeding, seizure management, and similar supports.

At Evolving Care, our support workers provide person-centred services that enhance our participants' independence, safety, and quality of life.

Key Responsibilities

Deliver daily supports in line with personalised care plans

Provide complex care where required, including (if trained):

PEG feeding

Seizure and epilepsy support

Complex bowel care

Wound and pressure care

Diabetes monitoring

Catheter management (IDC/SPC)

Tracheostomy care

Support daily living, home maintenance, and community access

Accurately document shift notes and report concerns

Follow Behaviour Support Plans as required

What We’re Looking For

Experience with personal care and (if applicable) complex health supports

Certificate III in Individual Support, Certificate IV in Disability, Mental Health, or related qualification (or be an RN/EN)

Ability to work independently and follow clinical instructions

Strong communication and time management skills

Current NDIS Worker Screening and Blue Card (or willingness to obtain)

Valid First Aid & CPR

National Police Check

Completion of:

NDIS Worker Orientation Module

NDIS Infection Control Module

Current driver’s licence and reliable, insured vehicle
