Companion Health is a registered NDIS provider based in Perth, committed to delivering high-quality, client-centred care for individuals with diverse needs throughout the Perth metropolitan area. As we continue to expand, we are seeking an experienced, dedicated Disability Support Workers to delivery of high-quality, person-centred support across our supported accommodation homes.
In this client-focused role, you will become part of a supportive and friendly team, providing care to participants with complex medical needs and behaviours in their own homes or shared accommodation. While this position is primarily based in the Byford area, flexibility to work in other locations may be required.
We are seeking candidates interested in part-time or full-time work, with availability for a variety of shifts—including: mornings, evenings, and overnight (both active and sleepover shifts). Due to client requirements, this role is open to female applicants only. To be considered, you must have a minimum of 12 months relevant experience, a valid Australian driver’s licence, and a reliable vehicle with comprehensive car insurance.
Key Responsibilities:
Follow individual support plans, behaviour support plans and health care plans
• Maintain and monitor a client’s person-centred support plan
• Personal care including assistance with showering and grooming
• Preparation of meals as required
• Assisting with medication administration
• Daily documenting client's activities through progress notes
• Cleaning, general housekeeping and household tasks
• Tidying up common areas, making beds and washing linen
• Other duties as required from time to time
The ideal candidate will have:
Demonstrated experience in the disability or NDIS sector, with a strong understanding of person-centred practices.
Excellent communication and interpersonal skills
A genuine passion to provide individualized supports
A can-do attitude
Good availability (mix of weekdays and weekends and nights) (Minimum of 3 days availability)
Proficiency in Microsoft Office
What You’ll Need:
Certificate III or Certificate IV in Disability or equivalent.
NDIS worker screening clearance
NDIS worker orientation certificate
Current Australian drivers license, reliable vehicle with comprehensive car insurance
Current First Aid, CPR, Manual handling and Medication Administration Certificates
