About the role
South West Healthcare (SHW) is seeking an experienced and compassionate Clinical Nurse Consultant - Care of Older Adults to join our Community Rehabilitation service
This dynamic role provides a proactive and preventative approach to care for older adults as inpatients and residents in aged care facilities who are experiencing clinical deterioration. You'll work closely with GPs and other healthcare professionals to assess, manage, and support older patients, helping them avoid unnecessary hospital admissions and maintain the best possible health outcomes.
Position overview
- Employment Type: Full Time (80 hours per fortnight with a monthly ADO), On-going position
- Location: Warrnambool Campus
- Classification: Clinical Nurse Consultant
- Salary Range: $116,760.80 - $122,907.20 pro rata, plus superannuation, based on skills and experience.

The successful applicant will demonstrate
- Advanced clinical skills in aged care, gerontology, or acute nursing.
- Strong communication and multidisciplinary collaboration skills.
- A passion for person-centred, proactive care of the older person.
- Conduct comprehensive, evidence-based assessments in collaboration with GPs.
- Recognise and escalate clinical deterioration to appropriate services.
- Deliver timely triage and treatment aligned with Residential in Reach referral pathways.
- Ensure care plans are actioned, documented, and clearly communicated to all stakeholders.
- Support smooth transitions between aged care and clinical settings.
- Advocate for residents' goals of care and preferences.
You'll also:
- Provide clinical leadership in inpatient settings, supporting patients experiencing delirium, frailty, or cognitive impairment.
- Play a vital role in reducing emergency presentations from Residential Aged Care Facilities
