Your key responsibilities will include:

• Conducting comprehensive onsite medication reviews for aged care residents.
• Collaborating with healthcare teams to provide evidence-based recommendations.
• Educating and supporting aged care staff and residents in medication management.
• Ensuring compliance with relevant medication policies, standards, and guidelines.

About You

We are seeking a pharmacist who is passionate about improving health outcomes for older Australians. You will have:

• Current registration with AHPRA.
• Strong communication and collaboration skills to work effectively within multidisciplinary teams.
• A self-motivated, organised, and patient-focused approach to your work.

Previous experience in aged care or clinical pharmacy is highly regarded but not essential, as comprehensive training and support will be provided.
