The Role

Adelaide PHN is looking for a dynamic and strategic Integration and Relationships Coordinator to play a pivotal role in shaping a more connected, responsive, and people-centred health system across metropolitan Adelaide.

What You’ll Do:

  • Build and strengthen key partnerships with SA Health’s LHNs, the Local Government Association, and other stakeholders.
  • Co-design solutions that reduce unnecessary hospital presentations by improving referral pathways between primary care and hospital services.
  • Support place-based initiatives focused on vulnerable communities, ensuring no one is left behind.
  • Drive data-informed planning aligned with our priority health areas to deliver real, on-the-ground impact.
  • Facilitate cross-sector collaboration and foster change across internal and external stakeholders to deliver integrated care.
  • Spot opportunities for innovation and integration across portfolios within Adelaide PHN.

Who You Are:

  • A relationship-builder and communicator who can engage a wide range of stakeholders.
  • A strategic thinker with an eye for systems improvement.
  • Comfortable navigating complex health environments and leading change.
  • Experienced in health, social services, or local government sectors.

You may have had experience in previous roles in the primary health sector such as: Senior Project Officer, Project Manager, Program Manager, Primary Health Care Manager, Health Services Manager, Partnerships Manager, Senior Manager Health.

This position is being offered as full time and is funded until June 2026. Future employment is subject to ongoing funding.

About You

The knowledge, skills, experience and qualifications you will need for this role are:

  • Tertiary qualifications in health or human services or management (or other relevant discipline); or at a minimum, extensive experience within a health organisation.
  • Significant demonstrated experience in building beneficial partnerships and professional relationships with a wide range of stakeholders, including health care providers/peak bodies, community services, government departments employers and the general public.
  • Be an exceptional team player with high level interpersonal and communication skills (both verbal and written)
  • Ability to establish and maintain effective partnerships with LHN staff, stakeholders and key partners;
  • Participate in broader health systems level partnerships and integration.
  • Support place-based initiatives and activities with a range of stakeholders;
  • Be able to identify problems, challenges and barriers, and recognise opportunities, and come up with potential solutions.
  • Demonstrate experience in building and influencing professional relationships with a wide range of stakeholders, community services, and local health networks.
  • Have extensive experience in establishing, coordinating, implementing and evaluating innovative strategies.
  • An innate ability to influence and support organisations and health professionals through change management will be highly regarded.
