About the Role
As a Care Coordinator, you’ll play a vital role in supporting clients with complex and chronic health needs. You’ll coordinate culturally appropriate care that connects clients to a range of healthcare services and supports under Closing the Gap initiatives.
Key Responsibilities
Coordinate services and support in line with each client’s care plan
Collaborate with GPs, nurses, allied health professionals, and specialists
Accurately manage and update client records in the MKAC client data system
Promote and implement relevant Closing the Gap strategies
Identify and address gaps in health services to complement general practice
Encourage client participation in chronic disease care planning
Conduct assessments and manage health conditions within your scope
Provide client transport to and from appointments as required
Essential Requirements
Minimum Certificate IV in a Health-related discipline (e.g. Aboriginal Health, Nursing, or Allied Health)
Current registration with relevant professional body (e.g. AHPRA, ACN, etc. as applicable)
Valid WA Driver’s Licence
Current or willingness to obtain a National Police Clearance (issued within 3 months)
Current or willingness to obtain a Working with Children Check
COVID-19 Vaccination Certificate (Medicare/MyGov/GP documentation)
