About the role
We are looking for nurses that have experience in community nursing, palliative care and HITH services. You will provide high-quality care to clients in their own homes.

Call on Clare offers a range of home-based support services, enabling our clients to live with dignity and independence in the comfort and security of their own homes. We are a private provider of tailor-made services including in-home care, palliative care, nursing care and a range of other support services in Melbourne. We have rigorously vetted experienced nurses and carers who share Call on Clare’s core values of Compassion, Quality and Trust. We are available across greater metropolitan Melbourne, Ballarat, Bendigo, Geelong, and the Mornington Peninsula.
These rewarding roles assist clients with services including:
Direct care to clients and their caregivers along the palliative care trajectory
Clinical assessment and intervention to manage symptoms
Administering medications and assessment and treatment of pain
Assistance with basic, complex, and critical care needs.
Access to 24 Support from Call on Clare when needed
As our Palliative Care clients require 24 Hour around the clock care we can offer 8-10 hour shifts that suit your availability and other work commitments from Monday to Sunday. We are flexible to work around your schedule and other commitments and aim to give you the number of hours you require.
For Nurses wishing to undertake general nursing / specialist nursing roles we are looking for some of the following clinical skills:
Complex Wound Dressings including (VACC Dressing's)
Central Line Management - Looking after ports/PICC/IV Anti's.
Catheter Care (M) & (F)
Drain Tube Management
Continence nurse specialist (continence assessments etc. )
Assessments ( PAS, Cognitive, Wound, FRAT)

About you:
You are a dedicated nurse with community nursing experience who enjoys working in the community to support people meet their health goals and objectives. You have excellent communication skills, are caring, compassionate and are a great team player.
Current DIV 1 / RN AHPRA Registration
Minimum of 2 years Palliative Care experience in a hospital setting is highly advantageous
Demonstrated professional knowledge, judgment, and skill in providing professional support to patients with complex needs
Excellent communication skills with a compassionate and empathetic approach to your work
Computer literate and able to use MS Office, email, and rostering systems
Current CPR and Manual Handling Certificate
Able to provide proof of COVID Vaccination
Victorian Drivers License
NDIS Worker Screening Check
