ABOUT THE ROLE
The Family Support Network (FSN) Coordinator is responsible for ensuring the effective day-to-day delivery of the FSN services, with a strong focus on managing the common entry point, coordinating referrals, conducting screenings and assessments, and facilitating the delivery of targeted supports. The role drives delivery of the Intensive Case Management (ICM) stream in line with the FSN model. A key part of this role is working collaboratively with FSN partners, and the Department of Communities (Communities).
About FSN:
Wungening leads the Armadale Cannington Family Support Network. Family Support Networks (FSNs) are a partnership of community sector services and the Department of Communities (Communities), providing a common entry point to services and delivering targeted support to families facing challenging situations and those most likely to be involved with the child protection system.
Responsibilities Overview:
Coordinate day to day operations of the Family Support Network, ensuring timely and effective delivery of both service streams, Assessment and Coordination and Intensive Case Management in line with the WAFSN Operating Framework.
Work closely with the Manager - Family Support Networks to ensure effective staffing aligned to the needs of the network and the families, children and young people we work with.
Deliver effective team processes including organising team meetings and providing line management and supervision.
Facilitate and participate in case discussions, care planning meetings, and collaborative practice forums to support holistic responses to family needs.
Coordinate effective case management processes, including monitoring caseloads and workflows, screening, assessment, care planning, outreach and home visitation.
Support continuous improvement by identifying service delivery challenges and contributing to practice development initiatives to enhance team capability in responding to behavioural, emotional, cultural and developmental needs using therapeutic and culturally secure methods.
Ensure all practices align with Wungening’s values, policies, procedures, and guidelines.
Participate in the organisations ‘On-call’ processes as part of a rotating roster.
ESSENTIAL CRITERIA
Commitment to empowering Aboriginal families, children and young people and their right to participate in discussions and decisions relating to the supports they receive.
Experience in Early Intervention and Family Services or relevant child protection work with Aboriginal families, children, and young people.
Demonstrated experience in a coordination role including line management and supervision and managing staff performance.
Experience working with trauma-informed, restorative, or therapeutic care models.
Commitment to achieve compliance, policy implementation, risk, and quality assurance in the FSN service environment.
Strong skills in incident management, crisis response, reporting, and safeguarding.
Excellent communication and interpersonal skills, with the ability to engage and collaborate across diverse stakeholders.
Problem-solving, conflict resolution, and decision-making abilities.
Sound computer skills, experience working with MS Office programs and data management systems
Degree in Social Work, Psychology, Human Services, or a related field (or significant equivalent experience).
