Clinical Care Coordinator

Key Responsibilities:
- Oversee the delivery of care services, ensuring they meet safety, effectiveness, and regulatory standards.
- Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
- Develop and review individualised care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
- Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
- Ensure accurate and timely documentation of care plans, incidents, and resident records.
Key Requirements:
- Experienced Registered Nurse with current AHPRA registration who is looking to take a step up in their career or an experienced CCC looking for a new opportunity.
- Previous experience within a CCC (or similar) leadership position preferred, but not essential.
- Strong leadership, organisational, and communication skills; ability to manage and motivate a team effectively.
- Knowledge and experience of aged care accreditation, regulations, standards, and best practices.
- Compassionate, proactive, and committed to high-quality resident care.
