About the Role
We’re seeking a dedicated and passionate HMR Pharmacist to join our multidisciplinary team. You’ll be embedded within the practice, with direct access to our doctors and patients – enabling more meaningful, timely, and effective medication reviews.

Your responsibilities will include:

Conducting comprehensive Home Medication Reviews (HMRs)

Collaborating closely with GPs to optimise patient therapy and reduce medication-related harm

Supporting medication management for high-risk patients, including those recently discharged from hospital

Educating patients and carers to promote safe, effective medication use

Participating in quality improvement initiatives and case conferences, as needed

Remuneration:

$100+ per hour, based on experience and performance (employee arrangement)

Contractor rates up to $200 per hour, depending on performance

Flexible engagement model – choose to join as an employee or contractor

What we’re looking for:

AHPRA-registered pharmacist with current accreditation to perform HMRs (AACP or equivalent)

Excellent communication and clinical problem-solving skills

Ability to work independently and within a multidisciplinary team

A passion for patient education and preventative health
