About the Role

HomeCaring Sunshine Coast is looking for a passionate NDIS Support Coordinator to join our growing team. In this rewarding role, you’ll work one-on-one with NDIS participants to connect them with the services and supports they need to reach their goals and live independently.

Key Responsibilities

Support clients to understand and implement their NDIS plans

Coordinate a range of services and providers in line with participants’ goals

Build capacity and confidence in participants to manage their supports

Monitor outcomes and maintain clear, professional documentation

Liaise with families, carers, and stakeholders to ensure high-quality care

What We’re Looking For

Strong knowledge of the NDIS and local disability services

Excellent communication, organisation and interpersonal skills

Empathetic, person-centred approach to client support

Qualifications in Social Work, Psychology, Community Services or similar (desirable)

Previous experience in support coordination, case management or disability services

Valid driver’s licence and reliable vehicle

NDIS Worker Screening Check and Blue Card (or willingness to obtain)
