Change Futures is a Brisbane based not-for-profit organisation providing Mental Health services to a range of community groups. Our mission is to work with people of all ages to provide opportunities for positive change and to enhance their psychological and emotional well-being.
We are seeking Social Workers and 4th/5th year Psychology graduates or Provisional Psychologists to join our dynamic team. The successful candidates will be motivated to grow their career and possess a high level of confidence, interpersonal and organisational skills. These roles will work autonomously in the community visiting Residential Aged Care homes and will be responsible for the delivery of the PAC Program (Psychology Aged Care) to older adults.
Current Vacancies - Mental Health Practitioners:
PAC Program - Brisbane South: Springwood, Algester, Eight Mile Plains and surrounding areas (part-time community-based roles are available)
PAC Program - Gold Coast: Tallebudgera, Elanora, Burleigh Heads and surrounding areas (part-time community-based roles are available)
PAC Program - North Coast NSW: Tweed Heads to Pottsville (part-time community-based role)
PAC Program - North Coast NSW: Byron Bay to Ballina (part-time community-based role)
You will be responsible for:
Delivery of our Psychology in Aged Care program, by providing assessment and psychological support to older adult residents living in Aged Care
Providing face to face evidence-based assessments and interventions to individuals/groups residing in Aged Care homes
Working collaboratively and effectively within a multidisciplinary team
Working with culturally sensitive practices for clients to deliver quality psychological services
Maintaining confidential session records and timely entry in client management systems
Composing detailed, concise correspondence to stakeholders (GP letters, risk reports)
Collaboratively working with agencies and allied health professionals
Developing clinical practice knowledge through active learning and engaging in professional development
Daily travel is an ongoing element of these roles as these opportunities are based in a variety of Aged Care/clinical settings within the larger PHN region and travel will only be required within the specific region that you work in.
About You:
Graduate / Graduating Social Worker or Psychology (Bachelor or Master’s Degree)
Eligible to apply for registration with either AASW or AHPRA
Experience in Educational, Disability and / or Community processes is preferred
Fully vaccinated in compliance with the Aged Care Industry
Own vehicle and Australian Driver’s license
