About the role

We are looking for a Senior Care Manager to provide operational/day to day leadership in the delivery of high-quality, person-centred care within the Support at Home program.

The position is accountable for the oversight, support, and professional development of Care Managers across designated region(s), ensuring service excellence, regulatory compliance, and continuous improvement in client outcomes. Reporting to the Team Leader of Home Care, the Senior Care Manager implements key initiatives in staff supervision, quality assurance, risk management, management of budget responsibilities and service delivery and evaluation, while fostering a culture of accountability, continuous learning, and client empowerment. Some of the main responsibilities of the role include:

  • Provide mentoring and day to day management to Care Managers within region/s and undertake regular staff supervision meetings and documentation.
  • Undertake annual career development reviews and ongoing performance management of Care Managers as required.
  • Support and advise Care Manager in strategies to manage clients presenting with complex needs.
  • Ensure all care management practices, ensure service delivery align with Support at Home program guidelines, relevant aged care standards, and organisational policies
  • Lead ongoing high risk client meetings and case conferences with relevant health professionals to ensure coordinated, person-centred care, where required.
  • Conduct regular reviews and audits of client care notes to ensure accuracy, consistency, and alignment with care plans and best practice standards.
  • Support care managers in empowering clients—and their carers or supporters—to make informed choices, including the right to take risks, aligned with the Supported Decision-Making Framework.

Qualifications

  • Tertiary, ideally Bachelor level, qualification in Social Work, Allied Health, or Nursing
  • Current APHRA registration would be highly regarded

Skills & Experience

  • Prior success in managing staff, ensuring a positive and supportive working culture
  • Strong staff leadership skills with the ability to drive a positive work culture as well as handle difficult situations as necessary.
  • Demonstrated case management experience within the sector, preferably working with complex family and social issues.
  • Thorough understanding of, and empathy for, the aging process, aged care systems and opportunities.
  • Ability to quickly and accurately communicate details about a client’s condition and ongoing care to clinicians, family, representatives and the MiCare team as appropriate.
  • Problem-solving capabilities to work around complex schedules or walk patients through secondary plans when first options fall through.
  • Clear understanding of the Aged Care Quality Standards.
