About the role

We are looking for a passionate and skilled Support Coordinator / Support Worker with 1-2 years of experience to join our friendly team at Diamond House Support Services, Clubhouse SA Inc. in Port Adelaide. In this role, you will be instrumental in delivering high-quality support and coordination services to our clients in a community mental health setting. Your work will provide essential NDIS support to individuals with psychosocial disabilities, empowering them to lead fulfilling lives.

What you'll be doing

Coordinating and delivering personalised NDIS support services to clients to help them achieve their goals and enhance their independence

Assisting clients with daily living activities, social and community engagement, and accessing relevant services and resources

Developing and maintaining positive relationships with clients, their families, and other stakeholders

Advocating on behalf of clients and Members to ensure their needs are met

Maintaining accurate records and documentation in line with NDIS and organisational regulatory requirements

Participating in Diamond House Support Services (NDIS) team meetings and contributing to the continuous improvement of our service delivery

What we're looking for

Relevant qualifications in Community Mental Health services (Min Cert IV), or a related field

Demonstrated experience in a similar support coordination / direct support role

Proficient in IT and Client Management Systems, excellent interpersonal and communication skills, with the ability to build trust and build rapport with clients

Strong problem-solving and decision-making skills, with a focus on person-centred and outcomes-based support

Familiarity with relevant legislation, Policies, and procedures in the community services sector including National Mental Health Standards, NDIS Safeguard Commision and Workplace Health & Safety.

A genuine passion for making a positive difference in the lives of people with diverse needs and backgrounds

Understanding of teamwork principles
