About the role

Permanent full time opportunity to provide contemporary evidence based consumer/patient focused care, in collaboration with the interprofessional health care team to facilitate optimal health outcomes for the recipients of healthcare services and undertake delegated activities while assuming individual accountability for actions within the position and context of practice.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Contribute to the development, implementation, and evaluation of referral pathways to Aboriginal and Torres Strait Islander Community and Community Controlled Health Services (ACCHS) and the Primary Health Networks (PHNs) ensuring continuum of care through follow ups and referrals.
  • Participate in the transformation of ETC into a culturally safe environment for staff, patients and visitors that enhances their experience and enables them to create links fostering connections with community.
  • Contribute to the development, implementation and evaluation of a Cultural Practice Program specific to the ETC.
