Case Manager
Canberra Police Community Youth Club is a not for profit organisation that engages young people and their families through a tiered approach of community engagement, early intervention and diversion programs. Our mission is to build positive futures for young people and their families in Canberra and its surrounding regions. PCYC is committed to seeing Canberra's most vulnerable people being safe, connected, empowered, and achieving lasting change. Canberra PCYC helps people achieve this by providing strength based, client centered support in the form of case management, mentoring, and tailored evidence based support programs.
We currently have 1 PTE (Part-Time Equivalent) Case Manager vacancy in Project Empower our case management program for 3 days (22.8 hours) a week. We are seeking appropriately skilled case managers with youth engagement experience and knowledge of the sector.
DUTY STATEMENT
In accordance with Directorate Policies
Manage the daily operations of the program including, but not limited to, processing and contacting new referrals, engaging with new and existing clients, and completing support plans and reviews
Participate in regular team meetings, staff supervision, and whole-team training as outlined in the program KPI’s
Work collaboratively with other program staff and Team Leaders to foster the growth and development of the program
Provide support to clients and respond to any issues that arise within the service delivery – related to clients, WHS and case management practices
Build organisation image by collaborating with stakeholders, community organisations and employees
Engage in community sector development as outlined in program/role KPI’s
Follow Canberra PCYC policies and procedures and ensure compliance
Follow risk management protocol to address any risks that may hinder the progress of the program
Complete all required data collection with clients, in a timely and professional manner
Maintain good communication and relationships with clients and other staff
Have strong organisation and time management systems to sustain the program and provide care to clients
SELECTION CRITERIA
1. Knowledge and experience in the following fields:
- Individual and family case management
- Client Advocacy
- Outreach support
- Collaborative professional practice Capabilities and Behaviours
2. Experience working effectively with vulnerable and disadvantaged people and those who have experienced trauma
3. Ability to prioritise workloads and meet internal and external deadlines, the ability to use initiative, work independently and use sound judgement and adhere to professional boundaries
4. Sound knowledge and understanding of current issues affecting young people and families.
5. Demonstrated skills in the use of Microsoft Office and reporting through data base collection systems.
6. Excellent interpersonal and communication skills including written skills and liaising with Government and other NFP organisations
MANDATORY
• Must have a current Working with Vulnerable People Registration (WWVP) through.
• Certificate/ Diploma in youth work, community services or equivalent/higher
• First Aid Certificate or a willingness to undertake appropriate training.
• Current full driver’s license
DESIRABLE
• 12+ months case management experience (highly desirable)
• Experience working in a youth engagement setting.
