We have an exciting opportunity for a dedicated and empathetic Support Coordinators / Psychosocial Recovery Coachs to join us as sub-contracters, supporting participants across South East Queensland.

Key responsibilities includes but not limited to:

Working closely with the participant, families, NDIA and other stakeholders such as; Office of the Public Guardian, Public Trustee, NDIS Service Providers and Community and Mainstream services (Housing, Health, Justice, Education)

Supporting participants to build their capacity in understanding their funding, implementing their plan and exercising choice and control in choosing their supports

Supporting the participants with an emphasis on their short and long-term goals whilst monitoring and reporting their outcomes

Support participants to resolve conflicts that arise

Provide written reports as required to support participant plan reviews

Gathering reports from services to support participant plan reviews

Respond to enquiries in a timely and professional manner

Provide ongoing individually tailored Support Coordination services

Have the capacity to network and develop positive relationships with local community groups, NDIS services and Mainstream services

Attend stakeholder and networking meetings

Requirements:

Tertiary qualifications in Allied Health, Human Services, Community Services, Disability, Mental Health or other related discipline

Have experience in the Disability/ Community Services field and demonstrate knowledge of the NDIS (Previous LAC’s and Support Coordinators encouraged to apply)

Experience supporting a broad range of participants with varying needs

Ability to work independently with minimal supervision

Possess problem solving, planning and organisational skills and be detail oriented

Current Class C Driver’s Licence

Current COVID-19 Vaccination Status

National Police Check

NDIS Disability Worker Screening Check

QLD Working with Children Check (Blue Card)

Access to a roadworthy vehicle with insurance and current registration

Must have working rights to work in Australia
