THE HUB
The St Vincent de Paul Society NSW has been awarded a grant from NSW Health to open and operate an Alcohol and Other Drug (AOD) Hub in the New England region, NSW. The Regional AOD Connection Hub, is an exciting new service that provides specialist AOD treatment and support, with a focus on delivering services tailored to meet the needs of people living in rural and regional areas. Collaborating with Armajun Aboriginal Health Service Aboriginal Corporation the AOD Hub provides services for people living in the New England region who are experiencing Drug and Alcohol concerns and other co-occurring needs.
THE ROLE
The AOD Care Navigator for Armajun Aboriginal health Service Aboriginal Corporation will support all aspects of the case management and care coordination journey with a particular focus on First Nations participants of the Hub. Partnering with our dedicated team members and external services, you will create person-centred support plans for each individual, rooted in empowerment and resilience, ensuring they receive the highest level of support services, and treatment.
Must be willing to travel.
RESPONSIBILITIES
Empower people to cultivate hope, empowerment, and a sense of connectedness
Assist in delivering psychoeducation and AOD group programs across diverse regions.
Support the Intake Screening process and conduct assessments for AOD Hub access.
Provide comprehensive case management and care coordination for AOD Hub clients.
Facilitate shared care arrangements to ensure collaborative services.
Conduct regular appointments using various modalities for AOD Hub clients. who live across the New England
Offer evidence-based treatment and personalised care plans.
Assess treatment outcomes and client satisfaction levels.
Tailor treatments to be culturally appropriate for Indigenous Australians.
Participate in daily intake and care coordination meetings & engage in multi-disciplinary case review meetings.
Act as a key worker in supporting, coordinating, and managing AOD Hub clients.
Liaise with referring agencies to ensure positive outcomes for clients
Develop and implement referral pathways for accessing community-based services
Provide support and care coordination for families and significant others of AOD Hub clients.
Maintain accurate client records and documentation.
YOU WILL NEED
Minimum TAFE Certificate IV in the welfare or related field.
Previous experience providing support to marginalised and vulnerable people
Demonstrated knowledge of issues relating to alcohol and other drug issues.
Demonstrated awareness of appropriate local community support services for alcohol and other drugs, including government agencies and NGOs, and the capacity to build and maintain strong relationships with such service providers.
Excellent written and oral communication skills
Ability to use sound judgement and act appropriately in crisis and emergency situations
Sound computer skills.
Ability to work independently and as part of a team.
Awareness and understanding of the Work Health and Safety legislation and a commitment to promoting safe work practices.
NSW driver licence
Willingness to complete a criminal history check, working with children and provide conformation of Aboriginality.
