Remuneration: $37.03 – $52.83 per hour

What you'll be doing
Join the TACP team to support older adults transitioning from hospital to home through client-centred rehabilitation and safe discharge planning. Your responsibilities will also include:
- Delivering evidence-based physiotherapy care
- Working closely with a multidisciplinary team
- Supporting clients to achieve functional goals
- Promoting safe and timely community discharge
- Contributing to quality and service improvement
This is a temporary full time position with the possibility of extension, offering an excellent opportunity to further develop your skills in a supportive and collaborative environment.

Essential Criteria:
- Qualification: Degree in Physiotherapy OR Equivalent
- Registration: APHRA Registration
Selection Criteria:
- Degree in Physiotherapy or equivalent, current registration with the Australian Health Practitioner Registration Agency (AHPRA) and current unrestricted NSW Drivers licence.
- Demonstrated Physiotherapy experience, including aged care and/or rehabilitation and experience delivering Physiotherapy in the community setting.
- Demonstrated computer literacy, high level verbal and written communication and interpersonal skills, including the ability to adapt communication style to suit different audiences.
- Demonstrated high level of organisational and time management skills.
- Demonstrated commitment to professional development, evidence based practice and continuous improvement.
- Demonstrated ability to work both independently and as a member of a multidisciplinary team.
- Demonstrated skill in the comprehensive assessment of older adults, and people with physical disabilities and the development of relevant care plans.
- Demonstrated ability to analyse problems, set priorities and apply a range of options to develop solutions
