Your Opportunity
Located in Dubbo, NSW, the AOD Case Worker will provide co-ordinated care and psychosocial support to individuals who are engaged in Alcohol and other drug treatment. You will engage in providing case management and case co-ordination to individuals with drug and alcohol needs including harm reduction in relation to substance use, housing, parenting and family support, income, and employment. You will be required to be creative in providing harm minimisation and relapse prevention strategies to support participants, to address issues and challenges that are faced throughout recovery to achieve their desired outcomes.
Your key responsibilities will be to:
- Respond to referrals of clients to the service from internal and external support services.
- Conduct intake assessments of suitability for support
- Maintain relationships with key stakeholders including clinical drug and alcohol treatment services including Far Western Local Health District and Non-Government Providers
- Work with clients to create individualized support plans addressing psychosocial needs.
- Provide ongoing case management sessions with clients and review progression.
- Assist clients in the process of transition out of the service into the community.
Requirements for Success
- Qualifications in Drug and Alcohol and /or Community Services or Case Management experience within a similar role
- Experience providing Case Management to individuals with complex needs.
- Understanding of substance use disorders and mental illness.
- Current NSW driver's license
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
