Health and Community Services (HCS) is dedicated to assisting individuals with disabilities in identifying the support that truly matters most to them. As a registered service provider under the National Disability Insurance Scheme (NDIS) with extensive experience in serving Victorians living with disabilities or developmental delays, including mental health challenges, we are well-versed in the NDIS and its impact on our participants.
We are currently seeking a Behaviour Support Practitioner with a strong understanding of NDIS legislation and standards. We are looking for individuals who are empathetic, genuinely care about participants, and are attentive listeners.
Successful candidates should possess the ability to evaluate their own work with a commitment to continuous improvement, striving to deliver the best possible outcomes for participants and their families.
Key Responsibilities:
Part-time will also be considered for this role
Support NDIS participants aged 16 to 65 years.
Conduct functional behaviour assessments and develop/implement Positive Behavior Support Plans.
Train and support families and stakeholders in implementing behaviour support strategies to reduce or eliminate behaviours of concern.
Monitor the effectiveness of behaviour support strategies.
Provide supervision to other PBS practitioners.
Previous NDIS experience will enhance your ability to achieve optimal outcomes for our customers.
Complete reports and work towards goals within NDIS plans.
Actively participate in team meetings and related training.
Qualifications and Experience:
Qualifications in Health Sciences or equivalent.
Experience in a Positive Behavior Support Practitioner role.
Deemed suitable by the NDIS Quality and Safeguards Commission to deliver Positive Behavior Support at the level of Core Practitioner and above.
A minimum of one year's experience in the role.
