The Role:

This exciting opportunity is focused on facilitating smooth transitions and maintaining regular contact with people moving into and already living in Specialist Disability Accommodation. Experience in the disability sector is highly regarded, with professionalism and integrity being a key component of this position.

Work from a boutique office in Collins Street CBD in an A-Grade building which has modern end-of-trip facilities.

As a Transition Lead, your role will include:

Facilitate transitions into Specialist Disability Accommodation, ensuring all paperwork and requirements are met prior to the move.

Maintain relationships with established stakeholders to ensure compliance and reporting is met.

Track progress of participants and support their SDA journey through the initial transition and ongoing.

Respond to any challenges and find resolution - address any issues with speed and efficiency.

Ensure clients have all information in preparation for a smooth transition.

Champion a ‘best customer experience’ approach to gathering insight and developing solutions for continuous improvement.

Complete a full handover of information to the Tenancy Management team.

Ensure stakeholder information is updated and maintained.

Work alongside the broader Operations Team to analyse inquiry data and identify trends to increase business opportunities.

Contribute to monthly reporting data and information.

Attend regular meetings with people's support teams ensuring accommodation features are appropriate.

Attend and represent InPlace Living at networking events.

Participate in public speaking opportunities at events and conferences.

Stay up to date with competitor trends and meet department KPI's.

Assist in the development of systems, processes and initiatives that enable InPlace Living to nurture and fulfil positive experiences with all relationships and stakeholders.

Travel regionally to connect with broader communities and maintain relationships.
