Liberty Health Services is a Registered NDIS provider established in 2019. We're established and managed by Nurses who focus on providing holistic and ethical care. We’re seeking an experienced NDIS Service/Care Coordinator to join the team.

The key purpose of this role is to manage and lead a team of dedicated support workers and team leaders to ensure our participants are receiving high-quality, person-centred care in supported accommodation and their community environments As well as managing the administrative work in the office. The role is permanent full-time.

Your Role:-

Welcome and onboard new clients, providing a clear explanation of our services and ensuring they feel confident in their decision to choose Liberty Health Services.

Work closely with the rostering team and support workers to organize and customize services that meet the unique needs of each client.

Conduct risk assessments and implement strategies to minimize potential risks, ensuring the safety and well-being of clients. Collaborate with clients' support providers, networks, and family members to deliver comprehensive care.

Address and resolve any client concerns or service-related issues promptly and effectively to maintain a positive experience.

Build relationships with stakeholders to enhance service delivery and growth.

Administration & Compliance:

Maintain accurate and up-to-date records of participant interactions, service agreements, and plan progress. As well as Employee files.

Ensure compliance with NDIS guidelines, policies, and procedures.

Prepare and submit reports as required, including progress reports and incident reports.

Continuous Improvement:

Identify opportunities for improving service delivery and participant outcomes.

Participate in professional development and training activities to stay informed about NDIS policies and best practices.

Contribute to a positive team environment by sharing knowledge, experiences, and resources with colleagues.

About you:

Registered and/or Enrolled Nurses will be highly regarded.

Demonstrated experience in rostering and coordinating the delivery of disability and/or community services;

Knowledge of NDIS Framework and Community Services Funding Models;

Working knowledge of best practice rostering, workforce optimization and system utilization;

Experience in managing workforce issues including supervision, coaching and mentoring;

Experience managing a complex disability or related Client presentations and complexities;

Experience in incident management and reporting;

Experience delivering outcomes and working with KPIs.

Support Coordination experience will be highly regarded;

Drive to grow in the role and organisation.
