About the Role

We are seeking an experienced and dynamic Self-Managed Home Care Manager to oversee a self-managed client base. In this leadership role, you'll manage operational functions, build strong client relationships, and coordinate a dedicated team to deliver exceptional care while respecting the autonomy of our clients.

This opportunity is best suited to a candidate with a growth mindset and the motivation to take charge of a rapidly scaling division. Your recommendations will be heard and you will have reign to develop the division.

Key Responsibilities

Build strong relationships with self-managed clients, offering guidance and support to help them maintain control over their care plans.

Collaborate with clients to design, implement, and adjust personalized care plans based on individual needs and preferences.

Work closely with caregivers, clients, and service providers to ensure timely, effective delivery of services.

Monitor service quality, promptly addressing any issues to uphold high standards.

Ensure all services meet legal and policy requirements, staying informed about the latest regulations.

What We're Looking For

Proven experience in home care package management is a MUST - Self-Managed Home Care experience will put you to top of the pack!

Strong knowledge of self-managed care models and person-centered practices

Exceptional communication skills with the ability to build rapport across diverse groups

Excellent organizational and time management skills

Leadership experience, with a demonstrated ability to inspire and manage teams
