The opportunity
Vacenti is seeking a full-time Transitions Coordinator to join our operations team at our Shared Services located at Upper Mount Gravatt, QLD. Reporting to, and working closely with the Clinical Systems Manger, this role will be responsible for supporting and streamlining a high-quality resident transition to and from our facilities. With a strong focus on best practice assessments and care planning for residents tailored to their needs, the role ensures timely pre-admission reviews, clinical approvals, and preparation for classification assessments. It also supports a smooth and well-coordinated transition experience for residents, promoting continuity of care.
Your main responsibilities will include:
Active assessment processes at the home(s) by coordinating direct resident care transitions in a timely manner while ensuring the highest quality of care for the residents.
Support pre-admission reviews, clinical approvals, and resident profile assessments to ensure appropriate care placement and preparedness for admission.
Assist in a high quality ‘Vacenti’ transition experience for residents, maintaining continuity of care and minimising disruption during their move into care.
Identify and make recommendations to identified areas for improvement and assist the Manager in the implementation of sustainable quality improvement initiatives.
Supporting residents, families, and teams as required
To be successful in this position you will need:
Experience in aged care funding systems and processes
Sound auditing, surveying, information gathering and collation and statistical analytical skills.
Current registration with AHPRA is required.
High level of competency with office management systems and MS office applications, including Word, Excel and Outlook.
