Job description - Registered Nurse/Care Coordinator

Bannister In Home Care is seeking to fill a full-time position for our S.E.Queensland & Northern NSW operation in a Registered Nurse/Care Coordination role. This role is suited to someone looking for responsibility and a challenge.

About Us

Bannister In Home Care family-owned service provider that was established in 2013. In 2017 we were approved as a provider of Home Care Packages and NDIS supports. We pride ourselves on delivering professional and affordable in-home aged and disability supports.

Our Queensland operation provides in home care services to people in the NSW Northern Rivers, Gold Coast, Brisbane and Sunshine Coast communities.

We are currently looking for staff who are friendly, positive and determined to make a difference to expand our clinical workforce in line to our commitment to delivering high quality care.

Your part in our Clinical Team

As our Registered Nurse/Care Coordinator you will be allocated a portfolio of clients to manage as well as providing clinical care and oversight for our other clients.

Registered Nurse/Care Coordinator responsibilities include but are not limited to:

Case managing our current Home Care Package, NDIS, private and insurance clients

Onboarding new Home Care Package, NDIS, private and insurance clients

Communicating regularly with clients, family members and advocates

Following up on client’s clinical issues, hazards and incidents

Collaborate with clients, support workers, GPs, families, allied health professionals and other relevant parties

Managing client budgets providing information and education to clients, families and advocates about available services including government funded services.

Preparing reports and submissions for support plan reviews for home care package upgrades.

Working with clients to promote independence and reablement.

Approve appropriate client requests.

Working collaboratively within BIHC to ensure continuous improvement.

Market the business to community and client organisations to promote business growth

Development, implementation, monitoring and review of care plans

Coordinate the provision of quality person-centred care according to the client’s needs, service/care plans and available funding in accordance with budget

Conducting comprehensive clinical assessments

Clinical in-home services include care plan assessment, medication, incontinence (including catheter changes), chronic disease, complex wound management, palliative care, and diabetes management

Assisting with the orientation of new staff and ongoing professional development

Support in ensuring the service meets all accreditation standards

General administration with respect to tasks performed

Qualifications and Experience

Bachelor of Nursing, or equivalent qualification

Current AHPRA Registration (no restrictions and unlimited work entitlements)

Minimum 1+ years experience working in the Aged Care or Disability sector preferably in the community

Minimum 1+ years experience in team leadership/management positions

Essential Criteria

Relevant case management experience supporting clients in the community

Have a sound knowledge of Aged Care Quality Standards

Have a sound knowledge of Home Care Package Program (HCP) Operational Guidelines

Have a sound knowledge of National Disability Insurance Scheme (NDIS) Quality and Safeguards Commission Practice Standards

Current NDIS Worker Screening card

Strong computer skills

Current NSW/QLD Drivers license & own reliable vehicle (company vehicle supplied after probationary period)

Current flu vaccination (or willingness to get)

Current COVID vaccinations

Full working rights in Australia

Work environment

Registered Nurse/Care Coordinators is based at our office in Tweed Heads, client homes/meeting sites. Must be able to drive anywhere in the Northern NSW, Gold Coast and Sunshine Coast regions.
