North Eastern Community Aged Care features two distinct areas, each designed to meet the specific care needs and preferences of our residents:
Campbell Wing nursing home that accommodates 61 residents
Payneham Wing Memory Support Unit that accommodates 23 residents, thoughtfully designed as a secure environment providing specialised dementia care.
About the Role
To support the ongoing enhancement of our aged care facility, we are seeking a passionate, experienced and highly capable Clinical Nurse to join our team. This full-time position operates Monday to Friday, with rostered hours designed to provide critical oversight during high-risk care periods and mealtimes, in collaboration with another Clinical Nurse:
Mon – 0800-1600hrs
Tues – 0800-1600hrs
Wed – 1000-1800hrs
Thurs – 1000-1800hrs
Fri – 0800-1600hrs
As the Level 2 Clinical Nurse you will lead, support and provide direction and delegation of duties to our team of dedicated Registered and Enrolled nurses, carers and lifestyle staff to mentor, guide and supervise the delivery of quality services, with the aim of enabling our residents to live their best lives while in our care.
You will work closely with our Care Managers with assessment and planning of both clinical and social needs for each resident, development of care plans, monitoring and auditing results and keeping accurate and appropriate records of care delivery. This role specifically looks for applicants with sound supervision and leadership skills that looks to deliver care that is set well above the minimum standard.
Expertise and enthusiasm in the following specific areas are highly valued:
Continence Management
Wound care
Palliative care
Medication Management
You will be required to work within the framework of the Aged Care Legislation and the Aged Care Quality and Safety Commission accreditation standards leading with strategic initiative and mentoring the team on a daily basis.
Minimum Criteria
Registered Nurse with AHPRA with a minimum of 3 years of aged care experience
Demonstrated experience that highlights the importance of clinical expertise and the small moments that make a significant impact, fostering meaningful relationships and making a difference in the lives of our residents
A commitment to providing tailored care that meets individual needs and preferences and enhances residents quality of life
Dedication to maintaining high standards of care by ensuring clinical quality compliance
Ability to support and assist all residents including those with confusing or challenging behaviours or from a CALD background
Compassionate and dedicated individual with a positive outlook on health & wellbeing, caring and supportive approach and a keen interest in contemporary aged care practices
Excellent communication skills, confidence in conducting case conferences with residents and families Outstanding documentation skills to ensure accurate, timely and person-centred care planning and reporting
Well-developed time management skills
Ability to lead a multicultural team providing encouragement, education, and mentorship to nursing and lifestyle/care staff
Ability to build professional relationships with General Practitioners and Allied Health Providers
Committed to maintaining a safe and harmonious environment to both residents and staff
Utilising initiative and experience, to support growth in our continuous improvement strategies
Ability to identify and manage risk and escalate care concerns where required
Actively participate in committee meetings understanding and knowing the key needs of residents
Desirable
Experience in preparing for an accreditation survey
Additional Qualifications in Gerontology, Palliative and or Dementia Care
Experience utilising electronic resident record systems such as MANAD, Painchek and Medi-Map software programs
Demonstrated proactive approach to quality improvement initiatives seeking to elevate the standards of care
