Role Overview:
We are seeking an experienced Registered Nurse (RN) with AHPRA registration to join our supportive team. This role will involve visiting various client sites to conduct aged care assessments and manage the development and implementation of individualised care plans for elderly clients. You will work closely with a multi-disciplinary team to ensure that each client receives the highest level of care tailored to their unique needs.
Key Responsibilities:
Aged Care Assessments:
Conduct comprehensive initial and ongoing aged care assessments for clients in their home environments.
Evaluate the physical, emotional, and psychological needs of elderly clients, considering their medical history, daily living activities, and safety requirements.
Identify potential health risks and make recommendations for improvements.
Liaise with the client, their family, and other healthcare providers to ensure a holistic approach to care.
Individualised Care Plans:
Develop, implement, and regularly review individualized care plans that align with each client's specific needs and preferences.
Collaborate with care teams, including doctors, social workers, and allied health professionals, to ensure the care plans reflect the most up-to-date medical advice and treatment.
Monitor and document progress towards care goals, adjusting plans as required to reflect changes in client health and circumstances.
Care Coordination and Support:
Oversee the day-to-day care provided by personal care assistants, support workers, and other team members.
Ensure that care workers are well-informed and adequately trained to deliver care in line with the established care plan.
Provide regular supervision and guidance to the team to ensure high standards of care are consistently met.
Client Advocacy:
Act as an advocate for clients, ensuring their voices are heard in relation to their care needs, preferences, and decisions.
Promote clients' independence and dignity, assisting them in making informed choices about their care.
Provide education and support to family members regarding care requirements, health conditions, and available resources.
Compliance and Documentation:
Maintain accurate and up-to-date client records, including care plans, assessments, progress notes, and communication logs.
Ensure compliance with relevant healthcare regulations, industry standards, and company policies.
Participate in audits and contribute to quality improvement initiatives.
Team Collaboration:
Work closely with the clinical care manager, case managers, and support staff to ensure the efficient delivery of services.
Participate in team meetings, training sessions, and ongoing professional development.
Contribute to fostering a positive, collaborative work environment that emphasizes respect, empathy, and teamwork.
Skills & Qualifications:
Essential Qualifications:
· Registered Nurse (RN) with current AHPRA registration.
· Minimum of 2 years’ experience working in aged care, with a focus on assessments and care planning.
· Sound knowledge of aged care standards, NDIS (if applicable), and relevant healthcare legislation.
· Strong clinical assessment skills with the ability to identify and address a range of health concerns in older adults.
· Excellent interpersonal skills, with the ability to communicate effectively with clients, families, and team members.
· Strong organisational skills and the ability to manage multiple client assessments and care plans.
· First aid & CPR qualifications
· NDIS clearance
Experience in a community-based care setting, particularly in-home care assessments is desirable.
Proficiency with electronic health records (EHR) and care planning software.
